Health Reimbursement Program

A component of the 2013-2015 Executive Branch contract is the added benefit of a new program offered by Anthem, which combined with the existing Health Assessment Tool (HAT) benefit, will provide up to $500 for health related expenses. We have received more detailed information about the program and are happy to share it with you.

Here is a document the NH Dept. of Administrative Services released about the program. This document provides much information and numerous links. We are summarizing some of the information in the paragraphs that follow.

By this time, you should have one Health Benefits Debit Card account that you will use to access your health promotion HRA dollars and your Flexible Spending Account (FSA), if you have one. If you already have a Debit Card, continue to use the same card(s) in 2014.

If you did not currently already have a card, you should have been mailed two cards from Combined Services, LLC. One is for you and the other is for your spouse or dependent, age 18 or older. First, in order to be eligible for the Health Rewards Program, you will need to complete the HAT.  You will find the HAT by going to and logging in. If you have not created a user name and password previously, you will have the option to do so by clicking on “Register Now.” Just follow the steps that are outlined for you.

Next, select the tab that says “Health & Wellness.” Then click on “Get Started” under the “Healthy Lifestyles” section.  Accept the terms and conditions and then click on “Submit.”

Next, select “Well-Being Assessment.” Then, click on “Start” and begin answering the questions and follow the rest of the steps as prompted.

Once you’ve completed the HAT, you are eligible to begin earning Rewards points by choosing up to a maximum of three activities of the six activities that will be available and redeem up to 300 points/$300 dollars in a calendar year. These are the six activities from which you can pick.

Know Your Numbers Health Screening – Complete a worksite screening or submit a physician fax form reporting important numbers like blood pressure, body mass index, glucose, and cholesterol. You can find out more about this here.

Participating in Fitness and Nutrition Activities – Each activity is worth five points. A fitness activity is defined as being physically active for at least 30 minutes a day on five or more days in a week. A nutrition activity is defined as consuming at least five servings of fruits and vegetables on five or more days in a week. You will earn 100 points after reporting 10 fitness events and 10 nutrition events.

Completing the Employee Health Education Program – Program modules include

  • Understanding Your Health
  • Medical Self Care
  • Health & Wellness 101

Modules can be completed in workplace presentations, online, or on paper.

Getting an Annual Check Up with a Health Provider – Get an annual checkup with a health care provider in 2014.

Getting a Flu Vaccination – get a flu shot in the fall of 2014

Tobacco Free – not using tobacco products

When do I get my rewards points/dollars?

If you have a Health Flexible Spending Account, funds will be available for deposit into your account on January 1, 2014.  If you complete the HAT, the $200 reward will be deposited into your Health Rewards account within 7 to 14 days. This benefit is available between January 1, 2014 and December 31, 2014. Once you have completed the HAT, you are eligible to receive additional rewards/dollars through certain activities. The reward/dollars associated with the activity will be deposited on your card within 7 to 14 days after completion of the activity.

What can I do with my rewards points/dollars?

The money deposited into your HRA and/or FSA accounts must be used for qualified medical expenses.

You can use your HRA for out-of-pocket expenses related to your medical and prescription drug coverage including HMO/POS co-payments, HMO/POS deductibles, coinsurance amounts incurred by you and your enrolled dependents. Effective January 1, eyewear and vision exams are included in qualified expenses.

Your FSA can be used for all IRS section 502 qualified medical expenses including the HRA eligible items previously mentioned and additional items such as dental expenses and some over the counter expenses. For a complete list click here.
