In order to be that, it’s important to check your height, weight, blood pressure, cholesterol, body mass index and glucose each year. Knowing these numbers can help you prevent health issues and learn where you can make improvements for your health and well-being.
A Know Your Numbers screening event will take place at the Department of Safety, 33 Hazen Dr., Concord in room 119 on July 19 from 9 a.m. to 3 p.m.
The screening takes about 15 minutes and there is no cost to you for participating. This screening is an activity that is part of the 2016 Anthem Health Rewards Program. All state employees enrolled in a State health plan through Anthem are eligible to participate in the Know Your Numbers screening event.
You can make your appointment by logging on at http://promotehealthyhabits.com/Signup/SONH/. Once logged into the Screening Self-Scheduling website click “Schedule Appointment”, then select “Safety – 7/19/2016 Screenings Concord” from the Facility/Location menu, reserve a time, and select an email reminder if you would like. If you are unable to log in to the system, you may schedule by phone, please call Healthways at 1-877-252-8410.
Your results will also be automatically uploaded to your Healthy Lifestyles Well-Being Health Assessment (HAT) if you have not completed your HAT. If you have already completed your HAT, the screening results will be uploaded automatically in the “Resources & Tools” tab in the Health Records section of your Healthy Lifestyles online program.
Your health numbers are secure and are not shared with anyone at the State of New Hampshire.
